                                            [0] => Introduction: In Colombia, 18% of the population lives in rural areas with limited access to healthcare services—a situation that was further exacerbated by the COVID-19 pandemic. In response, strategies such as telemedicine have been promoted. This study examined the perceptions of various health system stakeholders regarding the implementation of telemedicine for chronic disease management in a first-level hospital.
 Methods: A qualitative study guided by grounded theory was conducted through semi-structured interviews with 16 participants, including patients (n = 11) and healthcare professionals (n = 5), at a first-level hospital. The data were analyzed according to four major categories: opportunities, advantages, barriers, and limitations.
Results: Participants identified several opportunities, including improved access to medical care in remote areas, enhanced follow-up for chronic conditions, and more efficient scheduling of appointments and procedures. Advantages included increased accessibility and the ability to alternate between in-person and virtual consultations. Nonetheless, limitations such as insufficient infrastructure and lack of training were noted, along with barriers including distrust in technology and poor coordination among healthcare actors. 
Conclusions: The findings highlight significant opportunities and potential benefits for implementing telemedicine in first-level hospitals in Colombia. However, challenges remain, including inadequate infrastructure, limited training and resources, distrust in digital tools, restricted internet access, and a lack of coordination among healthcare stakeholders.
                                            [1] => Introducción: En Colombia, el 18 % de la población vive en áreas rurales con dificultades de acceso a servicios de salud, agudizadas por la pandemia de COVID-19. En respuesta a esta situación, se impulsó el desarrollo de estrategias como la telemedicina. Se planteó una investigación sobre la percepción de diversos actores de la salud acerca de la posibilidad de implementar la telemedicina para enfermedades crónicas en un hospital de primer nivel.
Métodos: Se realizó un estudio cualitativo orientado por la teoría fundamentada, mediante la aplicación de entrevistas semiestructuradas a una muestra de 16 participantes, incluyendo pacientes (n = 11) y funcionarios de la institución (n = 5), en un hospital de primer nivel. El análisis de los datos se realizó mediante categorías correspondientes a oportunidades, ventajas, barreras y limitaciones.
Resultados: Las oportunidades identificadas por los participantes incluyeron el acceso mejorado a la atención médica en áreas remotas, seguimiento de patologías crónicas y gestión eficiente de citas y procedimientos. Destacaron ventajas como la accesibilidad mejorada y la posibilidad de alternancia entre consultas presenciales y virtuales. Sin embargo, se argumentaron limitaciones, como la falta de infraestructura y capacitación, y barreras como la desconfianza en la tecnología y la coordinación insuficiente entre los actores de salud.
Conclusiones: Se destacan oportunidades y beneficios para implementar la telemedicina en un hospital de primer nivel en Colombia. Sin embargo, se señalan limitaciones como falta de infraestructura, capacitación y recursos, así como barreras como desconfianza en la tecnología, acceso limitado a internet y falta de coordinación entre actores de salud.
