                    [0] => The Institutionalization of Scientific Medicine in the fifth century a. C by Hippocrates of Cos, brought the need to register in other media - in addition to biological memory - the historical context of the patient (anamnesis) and the disease course and thus prescribe the possible therapeutic approaches to be adopted. Born there, the patient record, document consists of a set of verbal texts (narratives and reports) and nonverbal (x-rays, echos etc..) Regarding the ill person. By 1907, notes were made in a single document, in chronological order of registration, which hampered access to information when the return of a particular patient for continuity of care, noting, therefore, the need to individualize the charts. In research done to the mapping and analysis of the concepts adopted in nephropathic patients records of the university hospital (HUWC), Federal University of Ceará (UFC) in order to propose a categorization based on the terminology of specialized, aiming at building a taxonomic tree so that mirrors the conceptual representation of domain knowledge and thus contribute to a recovery of quality information. 50 records are digitized by mapping the concepts related to kidney diseases, inserting them into the Protégé software, according to the disease categories identified in the charts, with its many sub-clusters and hierarchical concepts related to it. Results show the representation of recorded knowledge in these documents, therefore, may offer clues indicators that could contribute to the access and retrieval of information.@en
                    [1] => A institucionalização da Medicina Científica no século V a. C por Hipócrates de Cós, trouxe à necessidade de registrar, em outros suportes - além da memória biológica - o quadro histórico do paciente (anamnese), bem como o curso da doença e, consequentemente, prescrever as possíveis condutas terapêuticas a serem adotadas. Nascia aí, o prontuário do paciente, documento constituído por um conjunto de textos verbais (narrativas e laudos) e não-verbais (radiografias, ressonâncias etc.) referentes à pessoa doente. Até 1907, as anotações eram feitas em documento único, em ordem cronológica de registro, o que dificultava o acesso às informações quando do retorno de um determinado paciente para a continuidade do tratamento, percebendo-se, portanto, a necessidade de individualizar os prontuários. Na pesquisa fez-se o mapeamento e a análise dos conceitos adotados em prontuários de pacientes nefropatas do Hospital Universitário Walter Cantídio (HUWC) da Universidade Federal do Ceará (UFC), a fim de propor uma categorização baseada nas terminologias de especialidades, visando à construção de uma árvore taxonômica de modo que espelhe a representação conceitual desse domínio de conhecimento e assim, contribua para uma recuperação de informação de qualidade. Digitalizou-se 50 prontuários, mapeando-se os conceitos relativos às nefropatias, inserindo-os no software Protégé, conforme as categorias de doenças identificadas nos prontuários, com suas inúmeras subcategorias, sendo agregados conceitos hierárquicos a ela relacionados. Os resultados evidenciam a representação do conhecimento registrado nesses documentos, por conseguinte, podem oferecer pistas indiciárias que poderão contribuir para o acesso e a recuperação da informação.@pt
